| Video Discription | This is a case of perimenopausal woman post total laparoscopic hysterectomy with bilateral salpingectomy for adenomyosis with histopathology positive for squamous cell carcinoma of cervix. As a part of completion surgery- complete parametrectomy with bilateral oophorectomy with bilateral pelvic lymph node dissection with infracolic omentectomy was done PET scan- mildly prominent left pelvic internal iliac lymph nodes of uncertain etiology or significance. No other abnormality or metabolically active lesion noted elsewhere. Intra operative findings: -Bladder and bowel adherent to vault -pelvic lymph nodes, peritoneal surface, omentum, bowel, diaphragm free from any deposits Key surgical points - dissection of spaces- retroperitoneal, para-rectal, rectovaginal, para-vesical space - dissection vesico-vaginal space by lateral approach above the plane of uterine artery - complete delineation of ureter till bladder, uterine artery till origin, deep uterine vein, infundibulo-pelvic ligament and uterosacral ligament till sacrum - identification of hypogastric nerve to preserve it |
