| Video Discription | Presenting an interesting case of laparoscopic VVF repair in a previously operated case of TLH in a patient with previous 3 LSCS. During TLH, the bladder was opened and repaired at the same time. The patient developed a leak after 10 days. We repaired the VVF after 45 days. Intraoperatively, the left-sided DJ stent migrated completely into the ureter, which was removed using a guide wire and stent pusher with the help of a 2.9 mm hysteroscope. As the patient had previous 3 LSCS, there was dense fibrosis in the vesico-vaginal plane — which was the most challenging part of this case. Another challenge was identifying the ureteric orifice after the DJ stent had migrated completely inside the ureter. We first inserted a Terumo guide wire, over which we advanced the stent pusher, and then guided the scope into the ureter. Since the VVF was close to the left ureteric orifice, we preferred to do DJ stenting to avoid ureteric injury while suturing the bladder.
