| Video Discription | A complex case of deep infiltrating endometriosis in a 38 year old patient with severe dysmenorrhea, dyspareunia, infertility and pelviabdominal pain. Preoperative work-up on MRI shows multiple endometriotic cysts in both ovaries, left parametrial nodule, utero-ovarian fibrotic adhesions and fibrotic thickening of the round ligament. Retro-uterine endometriotic fibrotic lesions with infiltration of the recto-sigmoid junction wall on a distance of 12 mm. Left retro-ovarian fibrotic lesions with infiltration of the sigmoid wall and an endometriotic fibrotic formation with hemorrhagic cysts infiltrating the wall approximately 4 cm long. Fibrotic lesions with multiple small hemorrhagic cysts inside (endometriosis) infiltrating the anterior wall of the uterus and the upper wall of the bladder, with right paramedian extension, adherent to the ovary. 1. What preoperative stage of endometriosis do you suspect ? 2. What surgical team would you assemble in order to provide complete excision of the endometriosic lesions ? 3. How would you counsel the patient ? |
