| Video Discription | Laparoscopic colorectal surgery in patients who have had open abdominal surgery previously can often be a demanding procedure, mainly due to dense adhesions and anatomical dislocations of organs. In such patients, more meticulous technique is required when entering the belly and dissecting adhesions. Oncologically safe resection is also more difficult to achieve in those patients. This video shows three examples of advanced laparoscopy in patients who underwent at least two previous laparotomies: Case #1: A 46 y.o. lady with rectal cancer. Has had two cesarean deliveries. Undergoing laparoscopic abdominoperineal resection after CRT. Case#2: A 44 y.o. gentleman with Crohn's disease for fifteen years. Underwent ileum resection in 2003 and ileocolic resection and anastomosis in 2009. Undergoing laparoscopic ileocolic resection again for a 4x11 cm mass located at the anastomosis site, leading to subileus. Case #3: 61 y.o. gentleman. Underwent anterior resection for sigmoid colon cancer in 2011, complicated by wound infection and ventral hernia. Had mesh hernia repair in 2012. Undergoing laparoscopic sigmoid colectomy for recurrent colon cancer. |
