| Video Discription | "Facial Nerve Monitoring Pitfalls" by Dr Jack Kartush presents causes and cures to prevent monitoring failures, whether from user errors or device malfunction. The history of the NIM Nerve Integrity Monitor is reviewed beginning with its conception in 1977 by Dr. Kartush. Monitoring has proven itself to be a powerful adjunct to reduce the risk of iatrogenic nerve injury during surgery. But "Poor monitoring is worse than no monitoring" - possibly leading the surgeon into greater harm. Recommendations are made to reduce user error by following Best Practices and a "Pre-flight" Checklist, just as pilots and surgeons are now required to do. The necessity to use "Active" monitoring with Stimulating Dissection Instruments is discussed to constantly map the location and integrity of the facial nerve, analogous to a minesweeper paradigm. This is in stark contrast to only passively responding to nerve injury warnings, or rare monopolar probe stimulation, when user error or nerve injury may have already occurred.
