| Video Discription | This 21-year-old male from Oman was born with multiple Tessier’s craniofacial clefts. He had a medical history of having undergone 14 surgeries previously for his complex skull and face defects in UAE, USA and Germany. Unfortunately the surgeries failed and he had severe secondary deformities. He had difficulty in eating and biting foods with his front teeth. Due to severely protruding and disproportionately large lower jaw, there was improper contact between upper and lower teeth. Due to the bilateral cleft lip and palate defect he had an alveolar cleft (gap in the teeth-bearing region) in the upper front jaw. There was a re-occurrence of oronasal fistula (gap in the roof of the mouth) due to which he suffered from regurgitation of foods from the mouth in to the nose. He came to our hospital seeking correction and craniofacial reconstruction. It was planned to do a facial cleft re-surgery, jaw correction and closure of the recurrent fistula. The chin bone was approached from inside the mouth itself. The excessive bone in the lower jaw front region was cut in to sections. The excessively prominent bone was removed (which is utilized as bone graft later in the surgery), the chin bone was reduced, pushed back and segments were fixed with bone plates and screws. With this jaw correction, the protruding and enlarged lower jaw bone was set back and reduced making the lower jaw more proportionate and aligned with the upper jaw. Bone plates and screws placed in the upper jaw in the previous surgeries done elsewhere were removed. The fistula defect in the upper front jaw was closed using a local flap. The excess bone removed from the chin was used as a graft to close the alveolar cleft defect. The bone was fixed with screws to close the bony gap in the teeth-bearing region of upper front jaw.
