| Video Discription | Surgical treatment of anterior skull base meningioma irradiated with gamma knife Streotactic radiosurgery is ongoing process to examine the efficacy and safety of meningeal tumors and the analysis of long-term outcomes. GC was used for high-risk meningiomas that are not suitable for a major surgical procedure or adjacent to critical neurovascular structures. Stereotactic radiosurgery is recommended for cavernous sinus meningiomas due to the high risk of surgery and possible complications. There is a consensus on removal of large meningiomas that having neurological deficit and edema. Although stereotactic radiosurgery is said to be a safe treatment method for meningiomas, its long-term outcomes are unknown. The most important advantage of surgery is that it gives the pathological diagnosis. Preoperatively, patients who have been detected radiologically as meningioma may be diagnosed as atypical-malignant meningioma, hemangiopericytoma, metastasis after postoperatively completing pathological examination. Such a situation disrupts the postoperative treatment algorithm. We present 6o years old male patient who had a mass in 2012 and received pimary gamma knife treatment in the outer center at same year. He was hospitalized due to an increase in the size and edema of the mass of the patient during follow-up. The cranial MR showed us extra axial lesions at the left frontobasal planum sphenoidale region (3.6x3.3x1.7 cm diameters) and in the right clinoidal region (1 cm diameter- appeared after gamma knife treatment) with homogeneous contrast. The patient was operated. The postoperative period was uneventful and the patient was discharged without any problem on the 4th day of the operation. Pathological diagnosis was determined as meningothelial meningioma who grade 1.
