MOHAMED MANSOOR., RESIDENT -- GENERAL SURGERY., APOLLO HOSPITALS., CHENNAI., INDIA. Total of 3 cases of level - 2 axillary clearance with breast conservation was done for early breast cancer. Method: Skin crease incision overlying the breast lump (4-5 cms) was made and a wide local excision of the tumour was done. Through the same incision, skin flap was raised up to anterior axillary fold and hemostasis was ensured. The da Vinci robotic cart was placed above the corresponding shoulder. The optic arm, right arm and left arm were placed through the same incision using a long bladed retractor to retract the superior flap. With the help of prograsp an the left arm and harmonic scalpel on the right arm, the anterior axillary fascia was opened and the entire axillary pad of fat was dissected upto axillary vein preserving thoraco -- dorsal nerve, intercostal-brachial nerve and the nerve to serratus anterior. A suction drain was placed in the axilla. The advantages of this technique were found to be: 1. Three dimensional view and magnification, thereby permitting a closer dissection and nerve sparing axillary dissection was possible. 2. Minimal blood loss. 3. Minimal postoperative drainage. 4. No seroma. 5. Early recovery and reduced postoperative neuralgia. |
