In this video, we explore the intricate removal of a 2.2-centimeter endometriotic nodule from the bladder detrusor muscle, pushing against the mucosa. Our patient is a 46-year-old woman with a history of hysterectomy for adenomyosis, who presented with constant pelvic pain and a feeling of incomplete evacuation. She had been treated multiple times for urinary tract infections. The procedure involves careful bladder mobilization and dissection of the bladder tissue from the endometriotic nodule without injuring the bladder mucosa. We use cystoscopy throughout the dissection to guide the removal and ensure bladder integrity. A stitch is placed on the nodule to maintain tension and aid in the removal of all fibrotic tissue. The nodule, located on the dome of the bladder away from the trigone, allows for easier dissection without concern for the ureteric openings. With the aid of the 10x magnified 3D vision provided by the robotic camera, and the 360-degree rotation capability of the robotic wrist instrument, we achieve complete dissection and removal of the endometriotic nodule. The final cystoscopic view confirms the successful removal of the entire nodule, with the bladder mucosa and serosa meticulously stitched to ensure healing. This video also provides insights into the incidence and involvement of urinary tract endometriosis, highlighting that bladder endometriosis accounts for 70-80% of cases.
