| Video Discription | In this rare surgical case, I performed a Lockwood’s intrainguinal repair for a missed femoral hernia in an 80-year-old lady who had undergone an inguinal mesh repair just two months earlier. At the time of her previous surgery, the femoral hernia was perhaps neither visible clinically, nor on ultrasound, nor on-table. This is not uncommon—femoral hernias are among the rarest hernias we encounter, often missed in the absence of classical signs and without anatomical exploration of the femoral canal during modern mesh repairs. In this video, I demonstrate a clean dissection and repair in a bloodless field, where a makeshift mesh plug was fashioned to occlude the femoral canal. A classic Lockwood’s approach was chosen over more extensive methods due to her age and the pain often associated with other repairs like Lotheissen or McVay. The lady was admitted in the morning, walked out in the evening—completely pain-free.
