Rate of recidivistic disease after traditional tympanomastoidectomy techniques can be upwards of 60%. This rate can be lowered to less than 3% with the addition of mastoid obliteration during canal wall up and canal wall down mastoidectomy. In this surgical video, the key surgical steps for epitympanum and mastoid obliteration are highlighted: - Cartilage graft harvest and processing - Separation of the aditus ad antrum and epitympanum from the tympanic cavity by a pallisade of cartilage grafts. This helps avoid spillage of bone pate or bone cement from mastoid bowl. - Obliteration of mastoid and epitympanum with bone pate (can substitute bone cement if needed) In canal wall down cases (not included in this video): - The mastoid tip is removed - A composite flap of cartilage and skin of the lower half of the concha is raised. This is then rotated to obliterate the lower part of the cavity. - Sinodural angle and mastoid antrum are obliterated by bone pate. - A pedicled myofascial flap is then used to cover the obliterated cavity In all cases, post operative antibiotics are recommended. |
