| Video Discription | This is a surgical video demonstarting the critical steps for lymphadenectomy done by laparoscopicaly which will be usefull for the gynaecologists,oncosurgeons,laparoscopists.PelvicLymphadenectomy(lymph node dissection) is an integral part of some gynaecological malignancy.executing it in proper way needs predictive anatomy as well as surgical skill as the lymph nodes use to remain in the vicinity of vessels of lower limb & deep pelvic vessels.Radicality is the basic principle to be followed in malignancy so en bloc dissection ensures the total clearance. lymphadectomy is necessery to remove the possible metastatic disease as well as properly staging the disease.Five groups of LN removed during pelvic lymphadenectomy-common iliac,upper external iliac,lower external iliac, obturator,internal iliac.for common iliac precaution to be taken not to injure the ureter & lower lumbar vessels.during external iliac dissection possibility of injuring ilio inguinal nerve ,genitofemoral nerve ,circumflex artery has to be protected.obturator LN removal has the possibility of damaging obturator nerve,obt. vessels,corona montis.Internal iliac group are intimately associated with int iliac vein so care to be taken not to injure.laparotomy is the surgical replica of laparoscopy & vice versa ,execution & approach depends on surgeon's discreation & gadgets availability.Hoever irrespective of method & approach best of the services must be rendered to patient of malignancy. thank u all |
