| Video Discription | We performed an Emergency Transposition of the left Common carotid artery to the left subclavian artery with Left carotid eversion endarterectomy for a 45 years old businessman, who presented with Crescendo TIAS, Global brain hypoperfusion, slurred speech, and right arm weakness, post mild flu symptoms. His COVID 19 swab is negative. CTA of great vessels of head and neck demonstrated total occlusion of the right internal carotid artery and total occlusion of the first 5 cm of the left common carotid artery. Duplex US scan documented 70% left ICA stenosis. We performed left eversion endarterectomy, first, then transposed the distal left common carotid under the sternomastoid muscle., to the left subclavian artery, in order to avoid using any synthetic material the CCA was transposed above the IJV. We divided the omohyoid muscle, mobilized the scalene pad of fat, sacrificed the clavicular head of the SCM, and the anterior scalene muscle after preserving the phrenic nerve, to get a bigger area exposed of left subclavian artery.
