| Video Discription | A 28-year-old female with known cholelithiasis presented with recurrent abdominal pain and febrile episodes for 10 days. Initial management at another center with intravenous fluids and high-dose antibiotics yielded no clinical improvement. On admission, she was hemodynamically unstable, severely anemic (Hb: 6 g/dL), oliguric, and exhibited leukocytosis (28,000/mm³) with thrombocytosis (platelets: 1,000,000/mm³). Imaging revealed Type III Mirizzi syndrome with bilateral cholangiolar abscesses, consistent with severe cholangitis and evolving multiorgan dysfunction. Broad-spectrum antimicrobials, thromboprophylaxis, and ERCP with biliary stenting were promptly instituted, along with blood transfusions. She was stabilized and discharged following clinical improvement. A month later, stent blockage necessitated replacement with a larger-caliber stent. Elective surgery revealed a densely inflamed gallbladder with dense adhesions to adjacent structures and a markedly edematous porta hepatis. A subtotal cholecystectomy with choledochoduodenostomy was performed due to hostile anatomy. Postoperative recovery was uneventful, with the patient discharged in stable condition. |
