| Video Discription | This is a 34-year-old woman Irregular menstrual cycle and spotting at due date for 2 months with no significant medical history.She has 3 normal previous pregnancies. This patient, using no contraception for the last 6 months, was referred to me because a mild pelvic pain and mild vaginal bleeding. The symptoms started 1 month ago and seemed to be increased in the last 15 days with recent deep dyspareunia and anal pain during defecation. Clinically, the lower abdomen was tender to touch. There was a small amount of vaginal bleeding. The uterus was in a normal size. There was a painful little mass in the cul-de-sacon bimanual examination. The serum HCG level was negative -less than4 UI/l, white blood cells were at 11,700 with 6.699 neutrophils, the C reactive protein was 16 normal less than6. Due to the moderate pelvic pain,Laparotomy was performed showing a hemoperitoneum with collection of pus in the POD with a intestinal coil like structure on left side of the uterus of the left tube. After salpingectomy, the histology revealed the presence of an ectopic pregnancy, associated with clots. The final diagnosis was left ectopic tubal pregnancy rupture with negative serum HCG level. |
