Delwar Hossain MBBS, FCPS (Medicine), FCPS (Gastroenterology) Senior Consultant - Gastroenterology SQUARE Hospitals Ltd. #Prof_Brig_Gen_Dr_Md_Delwar_Hossain Procedure: UGI Endoscopy + Clip Haemostasis Indication: Melaena under evaluation Medication: Under sedation Patient Age 69 year's Male FINDINGS: FOL: Epiglottis, aryepiglotic folds, piriform fossas and vocal cords appear normal. OESOPHAGUS: Multiple varix nodes are seen at the upper oesophagus. Rest of the oesophagus appears healthy. STOMACH: Mucosal erythemas are seen at the antrum, body and fundus. DUODENUM: A medium sized ulcer with active bleeding and adherent blood clots is seen at the D1. Haemostasis was done by applying Hemostasis Clipping Device (Micro-Tech) at the ulcer. There is no further bleeding is noted. Few small ulcers also seen at the apex of the bulb. D2 along with papilla appear normal. BIOPSY: Not taken. COMMENTS: Varix nodes, Gastritis, DU with active bleeding (Please see the description also). THERAPEUTIC INTERVENTION: Endoscopic clip haemostasis done. |
