| Video Discription | Brainstem cavernous malformations are uncommon lesions that can be associated with serious consequences. When bleeding, surgery is indicated to avoid the risk of rebleeding and to decompress brainstem structures. These are technically complex surgeries, that can lead to high risk situacions in terms of morbidity. To optimize the results of the procedure, it’s preferable to perform it during the first 8 weeks after the bleeding, as the bleeding itself creates a work plan between the gliotic tissue of the brainstem and the cavernous malformation. We present the case of a 65-year-old patient with a right medullary brainstem cavernous malformation. After bleeding, it caused right facial paresis (House-Brackmann grade III), diplopia, difficulty for swallowing fluids and unstable gait with lateralization to the right side. The operation was performed using the ORBEYE 4K 3D (Olympus) exoscopic visualization system, which facilitated the procedure with its hyper realistic image quality and the amazing ergonomics provided by its arm and its orbital camera (320°).
