| Video Discription | This video reviews the technique of robotic appendectomy, which includes port placement, instrumentation and four different approaches to perform the appendectomy. The four approaches are detailed as follows: 1. USING 30 MM STAPLER The appendix is localized, and robotic scissors are used to mobilize the cecum. The appendiceal mesentery is transected with a 30 mm Sureform grey stapler, then appendix is transected with a 30 mm SureForm blue stapler at its base. The appendix is placed in an endo bag for removal. The surgical field is irrigated with the robotic suction irrigation system. No bleeding in the staple line. 2. USING VESSEL SEALER AND 30 MM STAPLER The appendix is localized, and adhesions are separated. The appendix is freed from the retroperitoneum with the use of robotic vessel sealer. Transection of the appendiceal mesentery is performed with the vessel sealer. The appendix is transected with a 30 mm SureForm white stapler at its base. The appendix is placed in an endo bag for removal. The surgical field is irrigated with the robotic suction irrigation system. 3. USING MARYLAND DISSECTOR The appendix is localized, and a tunnel is created at its base of the appendix using Maryland dissector to prepare it for transection. The appendix is then transected at its base using a 30 mm SureForm white stapler. Transection of the appendiceal mesentery is performed with vessel sealer. The appendix is placed in an endo bag for removal. 4. USING SCISSORS The appendix is localized, and a tunnel is created at its base using robotic scissors to prepare it for transection. The appendix is transected with a 30 mm SureForm white stapler, the appendiceal mesentery is transected with a 30 mm Sureform grey stapler. The appendix is placed in an endo bag and removed from the abdomen. No bleeding from the staple line is noted. |
