| Video Discription | The patient, age 53 and in menopause, was told surgery was unnecessary. The insurance company wanted to deny her hysterectomy and recommended Lupron instead. Her MRI reported a normal rectal wall, no rectovaginal disease, and no adenomyosis. Despite this, intraoperative findings showed deep infiltrating endometriosis: the rectum was completely fused to the uterus, sacral ligament, and left ureter. The cul-de-sac was obliterated. There was rectovaginal disease with invasion into the interrectal wall, and the disease also involved the parametria and anterior vaginal wall. This was not subtle. It was extensive, invasive disease—entirely missed on imaging. Diagnosis was made through a rectovaginal exam and confirmed intraoperatively. Bowel prep was done, and a colorectal surgeon was present. Pathology confirmed deeply infiltrating endometriosis and adenomyosis.
