Medial to lateral dissection to open the Sylvian fissure in a Right fronto temporal exposure in a case of craniopharyngioma Few points are highlighted in this video – with the release of csf from the medial Sylvian fissure the brain becomes lax making the medial sylvian region more accessible and therefore providing the necessary space to dissect from proximal to distal. With the lax brain there is little or no use of a self retaining brain retractor.
I do not like any blood entering the sub arachnoid space so you can see that I am sucking the blood with the suction as it is trickling down from the dura which is flapped towards the base
The opening of the Sylvian fissure is carried out in a sharp manner with micro scissors. Also note that the sylvian fissure is in the center of the dural opening allowing adequate space on either side of the fissure - frontal and temporal for gentle handling / retraction. With smaller exposure the brain loses its handling property and the bone gets in the way - one cannot retract the brain against the bone or its cut margins |
