Male 42 years old, with a grade IV lesion without active blush or contrast extravasation seen at CE-CT scan (therero there were no indication for AE), which was mis-diagnosed by A&E docs at admission the day before because the patient had intellectual disability and did not report any history of abdominal trauma, Hb dropped significantly from 10.1 in the evening of admission to 7.6 the next morning, with stable hemodynamics. Urgent abdominal CT scan was requested and showed a large hemoperitoneum with a high grade spllenic injury. NOM was deemed to be contraindicated in such a patient because the patient was a mentally retarded guy living in a community (that is why he failed to report to A&E docs at admission any history of trauma but in retrospect the employers of the community thought he might have fallen or been hit in hypcondrium). The high grade of the lesion, withouth any Blush to be embolized, and the intellectual disability of the patient, were all factors considered a contraindication to NOM (patient is unreliable for Being observed, he was at risk of falling again, moreover he is living in a community but having no restrictions for going out e.g, to the supermarket or church or bar).
Procedure: Laparoscopic Splenectomy performed by a surgical trainee under senior supervision and mentoring (Attending Scrubbed in)
