Just zoom and see two left lateral wall big sessile fibroids with the omental adherence to one after the delivery of the uterus. Pampineform venous plexus predominant on Right side with engorged veins on lower segment, oblique lie ,mobile head and LEVOROTATED UTERUS IN SUCH CASES sometimes difficult to deliver the head one should identify the left round ligament pushing the uterus to right to equalise the incision on the LS .one need not clip the engorged veins but after incision , it was stretched gently with the fingers, engorged veins automatically will be lateralised without any damage. Omentum was released from the fibroid and both the fibroids were shaved at junction with the myometrium at coast of pseudo capsule ,raw area was approximated by figure of 8 stiches. Normal Posterior aspect well contracted and retracted uterus ( wrinkling of serosa and hard feel of the uterus) |
