POD,PARACOLIC GUTTER free from the deposits and the par aortic lymph nodes were not palpable . Since Dysgerminomas ( solid) mainly spreads by Lymphatics. See disparity with the Radiological interpretation. But one should learn to live symbiotically with them sharing the difference to do better in future but no one perfect . Here we did left side Overotomy ( sulphingo opherectomy) After this surgery Main complication is partial or total Tortion of the uterus ( gravid or non gravid) Now how to avoid this 1) the one which enlarges towards antimesentric boarder will have long pedical more chance of tortion and less chance of urateric injury. Here without severing the round ligament one can safely ligate the Infundibulopelvic ligament closer to the tumour laterally and medially tube with mesosulfinx and the ovarian ligament. To prevent tortion of uterus IP ligament stiched to the overian ligaments and plication or shortening of the round ligaments left to your discretion.. Urater runs along the base of the IP LIGAMENT. 2) If the mass enlarges towards the antimesentric border then the stump length is reduced, in a huge mass it may split the two leaves of the infundibulopelvic ligament and the leaves of the mesosigmoid colon and goes to the lateral pelvic wall then the urater will be close to the tumour wall. On table it may be difficult to get the Urologist to stunt the urater even if one stunt no guarantee that the urater may not injured. In such condition the round ligament is identified,cut and ligated then one can gets the adequate space to enucleate tumour with in its capsule like LIFTING THE DOSA FROM THE TAVA, the urater will goes away from the field of the surgery scalitanised overian vessels can be ligated comfortably. Skeletanised overian vessels. Finally the round ligament sutured to the adenaxial stumps of the uterus . Once nuckle of urater removed along with the mass while ligation of IP ligament it was recognised only after removal of the tumour it was repaired by end to end anstomosis with double J stunt without tension on suture’s line since sufficient length of the urater due to it’s stretching. Without any I am really sorry to say that thirty years back primi has under gone hysterectomy for the tortion of gangrenous unicornuate uterus in the 2nd trimester ? Low placed round ligament when uterus becomes abnormal organ . Now l felt undoing the torsion with HYSTEROTOMY AND WAITING FOR THE RECOVERY ❤️🩹 OF THE UTERINE VASCULARITY might have helped her . Pt was refered to the KIDWAI INSTITUTE OF ONCHOLOGY BANGLORE. |
