| Video Discription | Hysterectomy is the most commonly performed gynecological procedure and could be carried out abdominal, vaginal, laparoscopic or robotic with many variations. The first laparoscopic approach was described thirty years ago and over these three decades many changes and improvements were made in the surgical procedure. Typical descriptions of all minimally invasive techniques are with the use of uterine manipulators or other kind of vaginal tubes. The use of uterine manipulators is established in order to optimize surgical outcomes and minimize intraoperative complications, especially of the bladder and the ureter. However, after publication of LACC trial many concerns were raised over the use of manipulators during the surgical procedure. We have though expressed our concerns ten years ago, describing in detail our surgical approach for Total Laparoscopic Hysterectomy (TLH) exclusively without uttering manipulato. Of course an experienced and well-trained operative team is necessary to achieve satisfactory surgical outcome with low complications rates and short operating time. Our last studies evaluate the feasibility and safety of TLH without uterine manipulator in women with benign indications for hysterectomy.
