| Video Discription | This video shows the surgical technique for surgical embolectomy in acute ischemic disease, in a M1-M2 occlusion involving both M2 branches of the right MCA. The surgical treatment is considered as an alternative to a mechanical endovascular rescue if the conditions and materials are not quickly available. The surgery is made through a standard pterional approach, splitting the Sylvian fissure until the MCA bifurcation is exposed. The location of the thrombus is easily visualized due to the change of color of the artery. An arteriotomy is completed at the apex of the bifurcation. In cases of a thrombus in a linear segment, the arteriotomy is done transversal instead linear arrteriotomy to avoid postoperative stenosis. After extracting the thrombus, the potency is confirmed visually or with the use of the intraoperative Doppler or fluorescence techniques such as the ICG of Fluorescein videoangiography. The arteriotomy is closed with a 10-0 suture. |
