| Video Discription | After any endoscopic sinus surgery, wound healing takes time, and is an extremely important factor in achieving an optimal outcome. Wound healing in large part refers to “re-mucosalization” or re-lining of any exposed bony surfaces with a normal sinus tissue lining, called “mucosa.” During this healing process, exposed bone is first covered by blood clot mixed with early healing tissue, which we term an “eschar.” An eschar is basically a scab, and this plus dried out mucus that accumulates in the sinuses makes for a significant amount of crusting and debris that can build up in the nose after sinus surgery. The sinuses will not function and drain normally until all bony surfaces are covered with functioning mucosa, and this can take weeks to months. Until that point, patients are encouraged to flush their sinuses with saline solution, to remove any crusting that forms, and to moisturize the healing tissues to promote the healing process. Since the major variable in determining the duration of sinus wound healing is the amount of exposed bone at the end of surgery, surgeries that require extensive mucosal removal take longer to heal completely. For many sinus tumors, drilling is often required to remove thick bone blocking access to the tumor, but also to remove the bony base of a tumor’s attachment site. These types of surgeries commonly result in sinus cavities with substantial amounts of exposed bone, and therefore take more time to heal than with a standard sinus surgery. The modified endoscopic Denker’s approach is an example of a surgical approach often used to access and resect tumors in the maxillary sinus (cite). This video demonstrates the evolution of maxillary sinus wound healing from 2 weeks to 9 months after surgery. Patients vary in the exact amount of time they take to heal completely, but Rhinologists at Henry Ford follow these patients closely not only to detect tumor recurrence, but also to confirm their sinus cavities completely heal without sinus scarring. Complications from sinus scarring include abnormal mucus drainage that can become infected, nasal obstruction, tearing from the eye onto the face, or prevention of adequate tumor monitoring due to scar tissue blocking the endoscopic view of the tumor site. As can be seen in this video, the goals of postoperative sinus care in the setting of sinus tumor resection were achieved: postoperative monitoring for tumor recurrence was made easy by having a wide opening into the maxillary sinus without any scarring obstructing the view, and the sinus and tear duct remained open and functional. |
