| Video Discription | In this right ear, there is an epitympanic cholesteatoma extending into the posterior mesotympanum, retrotympanum and aditus. The malleus head, lenticular process of the incus and stapes crura are eroded. A totally endoscopic inside out (anterior approach: retrograde approach) canal wall down mastoidectomy was performed, except for a few minutes during which I swithced to the microscope for a more quick bone work to romove the posterior canal wall. An autograft TORP shaped bone is used to create an columella and transfer sound form the cartilage grafted new ear deum to the mobile stapes footplate.
