Although time is irrelevant, this serves to demonstrate that lap suturing is simple and achievable. My secret? My dependable useful lap trainer! Laparoscopic Vault suturing is a one of the key steps that determines whether a hysterectomy can be classified as total laparoscopic hysterectomy or laparoscopic assisted vaginal hysterectomy. I have witnessed the progressive transition from vaginal to total laparoscopic hysterectomy, having worked in two distinct countries. A meta-analysis (performed by Stefano Uccella published in #JMIG, 2021) revealed that barbed suture and laparoscopic method decreased vault dehiscence. I love laparoscopic suturing because the magnification I obtain for my vault closures is unparalleled; I can see all the layers of the vault and incorporate them, which, in my humble view, results in less vault haematomas. Although I have had training in both vertical and horizontal closure, and there is some evidence that vertical closure may result in an increase in vaginal length, I must admit that horizontal closure is my go to technique.
