| Video Discription | Bottom-up approach for eTEP Advantages: 1) No cross over is required so - technically easier to divide the PRS - inadvertent injury to linea alba can be avoided - Intraperitoneal gas doesn't affect the TEP space so struggle is not there 2) Important suturing of Diastasis is easier from leg ends (compared to head end) Other points due to our modifications: 1) Diagnostic lap allows the freeing of bowel loops (if present) so chance of inadvertent injury during blind dissection of hernia sac of regular eTEP technique is avoided 2) Placement of 5mm trocars in TEP space under vision 3) We use 5mm telescope so interchangeability of trocars for telescope is possible 4) Larger trocar is required for the mesh placement only so we placed it only at the site of hernia defect. We believe this way we can reduce new possible hernia sites |
