| Video Discription | Patient: Middle aged man with rectal bleeding for several years. He ascribed them to hemorrhoids and did not get screened when he turned 50 years or get investigated promptly when the bleeding persisted. Preparation: Split dose preparation with PEG solution Colonoscopy: Adult colonoscope with a cap Findings: Ulcerated friable mass obstructing the upper rectum. Biopsies: Multiple biopsies were taken to confirm the diagnosis. Also taking a biopsy from normal rectum before taking biopsies from the tumor helps in performing genetic studies to personalize care. Follow-up: CT colography done the same evening to rule out any other polyps or cancer in the remaining colon not examined by colonoscopy along with CT abdomen and chest to rule out metastasis. Plan: Consult multidisciplinary team of colorectal surgeons, radiotherapists and oncologists. Advice: Stay on full liquid diet and milke of magnesia to keep stools soft and liquidy and avoid obstruction. Stent deferred to avoid stent related complications as he does not clinical features of colon obstruction. |
