| Video Discription | For the first time in Iraq: Extracranial-Intracranial Bypass surgery for adult Moyamoya disease. A 20-year-old female with a history of stroke and TIAs. Imaging: stage 4 Suzuki with the absence of carotid terminus bilaterally and intense basal Moyamoya vessels (mainly perimesencephalic). The technique was not perfect. However, it was fair for the first attempt and the anastomosis checked via colored doppler. The whole procedure was under intraoperative neurophysiological monitoring (SSEP & EEG). Donor: STA (frontal branch) Recipient: M4 (posterior temporal branch). Type of bypass: Direct EC-IC. Time: 7 hours. Sutures: 10.0 prolene (simple interrupted) The post-Op course was uneventful.
