| Video Discription | Unicornute uterus with rudimentary horn and multiple fibroids The above surgery was started with an indication of adnexal mass with infertility. The mass was initially suspected to be a fibroid. Other smaller fibroids were also noted on ultrasound examination prior to surgery. Intra operative findings showed Unicornuate uterus with non communicating horn. The rudimentary horn had a large fundal fibroid and two smaller fibroids. The unicornuate uterus also had multiple fibroids. This is an interesting case which demonstrates how the findings may change intra operatively and that the surgeon must be prepared to face surprises during surgery. This surgery demonstrates the lateral window technique of bladder dissection that can be used in cases with previous cesarean hysterectomy. This surgery was performed by Dr Mayur Pai who is a laparoscopic gynec surgeon in Goa India. How to prepare for laparoscopic fibroid removal surgery? We prefer that the patient is on liquid diet the day before the surgery. This prevents any chances of constipation and in our experience, helps the patient recover faster. The lady is hospitalized the night prior to surgery. No the day of the surgery, the patient is nil by mouth and may receive mild sedatives to help her be comfortable. Anaesthesia for laparoscopic fibroid removal: General anaesthesia offers the patient full body anaesthesia. The patient is put to sleep with a drug and she will stay asleep and unaware throughout the procedure. The patient is fully conscious within minutes of the surgery and is able to walk on the same day. Exercise after fibroid removal: The patient does not have any abdominal scar and can resume her routine walks after 6 weeks of surgery. She need not take bed rest all the time and can resume her daily routine at home immediately after discharge from the hospital. Most ladies can drive a car and go to desk work after 7 to 10 days of surgery. Fertility after fibroid removal surgery: The fertility is seen to improve lots after fibroid removal surgery. Ladies are fit to have sexual activity within 2 to 3 months of surgery. And can begin infertility treatment after 6 months of fibroid removal.
