| Video Discription | Fluorescein is a fluorescent green dye that can be injected intrathecally (into the subarachnoid space in the lower lumbar spinal canal), and then circulates up to the brain cavity. If there are defects in the bony skull base (separates the sinus and brain cavities), and the outer meningeal layer (dura), then fluorescein will leak from the subarachnoid space in the brain cavity into the sinus cavities. This helps with identifying the skull base and dural defects intraoperatively, and helps confirm a successful seal of the reconstructed skull base at the conclusion of the case. In the case presented here, the patient was leaking cerebrospinal fluid (CSF) from his nose, but it was unclear preoperatively where exactly the CSF was leaking from. One technique to identify the leak site is to inject fluorescein as described above, and see where it leaks from in the sinus or nasal cavity. This case was unique in that CSF was found pooling in one sinus, but only because it was draining there from another area of the sinus cavities. Fluorescein made this very clear, and therefore made the case significantly easier, and contributed to the successful outcome. |
