| Video Discription | Eyelid Trauma Repair Oculoplastics Lid A short step-by-step video on lid margin closure as promised: Major tips: 1. Use a microscope if at all possible! 2. Ensure all your tarsus sutures are NOT full thickness - they need to be under the conjunctiva, otherwise you will get a nasty corneal abrasion 3. As you tie your sutures off - have your assistant pull the lid together to take off the tension until 2-3 sutures have been tied - this ensures the tension is neutralised and knots are tied tight . Step 1: Clear any fibrin from wound edges if wound not fresh, and ensure the tarsus defect is FULL THICKNESS - eg the full height of tarsus needs to be missing otherwise the edges will not come together nicely . Step 2: Place your 2 alignment sutures: the diagonal suture followed by the grey line sutures - ties these on a bow to ensure you are happy with alignment before going onto the remaining tarsus sutures . Step 3: Once all your tarsus-tarsus sutures are placed - tie the sutures off one by one, from lid margin downwards - leaving the ends long for the margin sutures . Step 4: Place a burried orbicularis suture below the tarsus defect - this pushes the lid margin UP helping to avoid a notch . Step 5: When closing skin, take a bite of orbic underneath - this avoids the orbic pulling the skin apart during healing . Step 6: Tie your lid margin suture ends under one of your skin sutures to pull the suture ends away from the globe . Sutures are all 6-0 Vicryl, and the skin sutures are removed at 2 weeks post op. |
