| Video Discription | 28 years Male presented with seizures. CT head showed AVM, which was evaluated by CT Angiography and MRI brain. Nidus was 5cm size, present at eloquent area and draining into both superficial and deep veins (Grade 4). It was supplied by left MCA (M4) branches and PCA (P2) branch. It was draining into transverse sinus, superior sagittal sinus and into straight sinus. I did excision of nidus along with my team and anaesthesia support. special points - Make image of nidus in mind using angio and MRI brain films - Try to locate feeder as per imagination and angio film - Proceed circumferentially (superficially), then go deep. - If bleeding occurs, focus on it. Before complete control, never proceed further. - Use cautery, aneurysm clip, surgical, cotton patty to stop bleeding - Save venous drainage for last - Deeper supply is always difficult due to choroidal arteries. Before coagulation, expose some length - Use non sticky bipolar or continuously irrigate it with saline.
