| Video Discription | . . Contrary to the movies, most surgeries are done with electrocautery, or some variation of that, and not with a scalpel. In general surgery, a scalpel is mainly used to incise the skin, and that’s it. . We cut and dissect soft tissues (fatty tissue, fascia, muscle, etc.) with electrocautery. This allows for a cleaner, safer, less bloody operation. . This particular device is a ligasure. It comes in different sizes. The two jaws close, with soft tissue between it. Using bipolar energy, the heat that is generated seals the tissue together. There is a blade in the middle that cuts through the tissue to release the two welded ends of that tissue. . This is very helpful for hemorrhoid surgery, and dissecting tissue that has a lot of blood vessels, like the incarcerated omentum in this ventral hernia. In the old days, we would clamp a blood vessel distant and proximally, cut between the clamps, then tie off each end. Doing this 100 times in a surgery takes up a lot of time. . . . |
