| Video Discription | When you hydrodissect, do you struggle with getting a dissection wave? Do you have to find different spot to push a bunch of BSS just hoping that one of them will catch a wave? Do you inadvertently get a delineation wave sometimes? Well, I present to you an easy and simple approach to getting a dissection wave every single time. Traditional techniques place the cannula just around the anterior capsular opening. The problem with this approach is that the fluid will follow the path of least resistance which is the pathway it came from, anteriorly and out the eye. This leads to the use of a lot of BSS and having to place the cannula in multiple places. This is rather inefficient and can be frustrating. My technique: I use a right angled Chang cannula (UPDATE: I developed my own cannula through Katena, the Kim Capsular Fornix Cannula which is longer with a blunt, rounded and safe tip) and I place it out to the equator and point the tip downward in the same fashion as I would with my chopper during horizontal chop technique. When BSS is injected, a dissection wave will occur every single time. And the reason is because the cannula is away from the anterior capsular opening and this prevents BSS from the cannula to find its way anteriorly. As a result, the only place the fluid can go is posteriorly which results in the dissection wave. |
