Huge hard bone focus filling the oval window. Combined techique of Hand-held CO2 laser (Omniguide) and skeeter drill stapedotomy. The drilling out procedure of the oval window usually requires a long time and is more difficult. Vein graft interposition. Ossiculoplasty with a 0.4mm diameter teflon piston. Transcanal approach. FRENCH: Technique chirurgicale pour otospongiose oblitérante (incidence de 3% dans notre série publiée de chirurgie primaire pour otospongiose - Otology & Neurotology Journal). Important foyer d'otospongiose envahissant la totalité de la fosse ovale et épaississant la platine. Technique combinée de stapédotomie au laser avec fibre (Omniguide) et micro-fraise diamantée. Le fraisage de la platine est difficile et demande un temps plus long que la normale. Interposition veineuse. Ossiculoplastie par piston Téflon de 0,4mm. Abord trans-canalaire.
