| Video Discription | This video illustrates surgical resection of intramedullary spinal cord tumor (IMSCT). The pathology of the tumor is EPENDYMOMA. This patient presented with several month history of balance problems, frequent falls, worsening arm and leg weakness and sensory changes. An MRI demonstrated large intramedullary (inside the spinal cord) tumor producing severe compression of the cord and accumulation of the cerebrospinal fluid (syrinx). The treatment of most spinal cord tumors is primarily surgical. The goal of surgery is to remove as much of the tumor as safely possible, ideally, the entire lesion. Spinal cord ependymomas are benign (low grade, or non-aggressive), but can be very large at presentation. Complete resection can be achieved in more than 80% of the patients. This tumor can come back in about 10% of cases. Neurological worsening is observed in about 50% of the patients with tumors in cervical and thoracic spinal cord. Other possible complications include cerebrospinal fluid leak and development of fluid collections in the area of surgery. Some patients may develop kyphotic (hunchback) deformity and require surgical correction. Most patients with WHO grade 2 ependymomas receive radiation therapy after surgery. The recovery after spinal cord tumor surgery make take some time. Most patients experience quite a bit of pain in the first few days and need to be treated with medications. The first 2 weeks are rough, but every day is a little better than the day before. Most patients return back to more or less comfortable level within a month or two after the procedure. Patients with neurological complication/damage may need rehabilitation. Most neurological improvement is observed within the first 3 month after tumor removal, and after about a year, most patients reach the plateau. |
