| Video Discription | Endometriosis, the presence of endometrial tissue outside of the uterine cavity, is a common cause of pelvic pain and infertility, affecting 10% of women. Endometriosis frequently affects the ovaries, where it causes cysts called endometriomas. Our group has described two types of endometriomas. Type I endometriomas arise from implants of endometrial tissue on the ovaries with subsequent invagination and bleeding into the ovarian stroma. In contrast, type II endometriomas arise from invasion of endometriosis into a previously existing functional cyst. Conservative management of endometrioma with hormonal therapy or with surgical cyst aspiration results in high rates of recurrence. Endometriomas should be surgically excised to treat pelvic pain and prevent infertility. In this video presentation, we demonstrate a minimally invasive, laparoscopic approach to excision of Type I and Type II endometriomas. Type I endometriomas are smaller, often less than 2cm, with more densely adherent capsules due to associated fibrosis and adhesions. Type II endometriomas are larger, with a clearer plane between the cyst and the ovary, facilitating easier stripping of the cyst from the ovary. Minimally invasive surgery for excision of endometriomas is an ideal way to manage endometriomas. In both Type I and Type II endometrioma excisions, we take care to remove the endometriomas in their entirely to prevent recurrence, while persevering the ovarian tissue as much as possible to protect ovarian reserve. |
