| Video Discription | Splenic hydatid cysts usually coexist with hepatic cysts, the most common form of hydatid disease; however, isolated splenic cysts are a rare entity that may lead to life threatening complications such as rupture associated with massive hemorrhage or anaphylaxis. Typically, splenectomy was considered the treatment of choice in especially large cysts and produces near complete atrophy of the splenic parenchyma; partial splenectomy has emerged as an alternative when there is a significant amount of splenic parenchyma can be preserved. However, these procedures are associated with hemorrhage, pancreatic fistula formation, and overwhelming post-splenectomy infection and sepsis. Other techniques such as fenestration and evacuation of the cyst as well as filling the cyst with omentum are often associated with notable complications. Through this case, we present a large isolated splenic hytatid cyst with a novel approach to the laparoscopic management. This is a minimally invasive technique through which we were able to resect pericystic and intracystic components while preserving the spleen with improved clinical and surgical outcomes. |
