| Video Discription | Total Laparoscopic Hysterectomy Being demonstrated by Gynecologist at Pai Hospital in Goa, India. Hysterectomy is the removal of the uterus. This surgery is now a days performed by laparoscopy of key hole surgery method. The patient has multiple benefits like lesser pain, lesser blood loss, and rapid recovery. The patient in most cases can be discharged on the same day or the next day. Laparoscopic hysterectomy is being demonstrated here using two different cutting instruments. The first instrument that you are able to see is called the Laparoscopic Shearer. Laparoscopic shearer uses bipolar energy to dissect. the second instrument that you see is called the harmonic scalpel. The harmonic scalpel uses ultrasonic energy to dissect. Both laparoscopic dissectors are extremely helpful in performing a laparoscopic hysterectomy and other laparoscopic surgery. Procedure of Laparoscopic hysterectomy: The upper pedicles are the round ligament, fallopian tubes and the ovarian ligament. These ligaments are individually sealed with laparoscopic bipolar and cut using a laparoscopic dissector. Then the two leaves of the broad ligament are opened and peritoneum is dissected anteriorly and posteriorly. Anteriorly, the bladder is dissected off the uterus. The uterine arteries are the sealed with the laparoscopic bipolar (yellow instrument) and cut using dissector. The uterine arteries are the main blood supply of the uterus. This is the main step in hysterectomy. Then, the utererosacral and cardinal ligaments are cut. The vaginal vault is opened and the uterus is removed vaginally. The suturing of vaginal vault is completed and bleeding areas are rechecked. Recovery from Laparoscopic hysterectomy: Laparoscopic hysterectomy or key hole hysterectomy has several benefits like early recovery, less pain and no large cut on the abdomen. Most ladies are fit to walk, go to bathroom and have a bath on the first day. Ladies can go to work and drive a car in 7 to 10 days. Sexual activity can resume in 2 to 3 month time. Heavy weights and exercise can be resumed in 2 to 3 months time.
