๐๐๐๐๐๐๐ ๐๐จ๐ฌ๐ข๐ญ๐ข๐จ๐ง- Lithotomy, arms tucked by the side ๐๐จ๐ซ๐ญ- โข First port- Camera (10 mm)- umbilical โข Second port- Right lower quadrant (12 mm) โข Third port- Right upper quadrant (5 mm) โข Fourth port- Left flank (5 mm) โข Fifth port- Suprapubic (5 mm) ๐๐ญ๐๐ฉ๐ฌ- In females uterus is retracted by stitching it to anterior abdominal wall. Dissection is started posteriorly. The plane between mesorectum and retroperitoneum is identified. The retroperitoneum is whiter than mesorectum. An energy device (harmonic/ligasure/enseal) is used to enter the posterior pelvic plane under the superior rectal artery. The left ureter and hypogastric nerve plexus are identified; dissection is extended downward through the presacral anatomic space, all the way to the pelvic floor. Dissection must be anterior to Waldeyer fascia. To facilitate exposure, the right lateral stock of the rectum can be mobilized. Now dissection proceeds anteriorly into the rectovaginal plane. This location often contains a hernia sac; retracting the rectum cephalad and cervix anteriorly facilitates exposure. Anteriorly rectum is mobilized upto the upper limit of vagina. The nervi erigentes and left lateral ligament are spared. A small rectangular sheet of polypropylene mesh is inserted via the right lower quadrant port and placed from pelvic floor, cranially behind the mesorectum. 5 mm suprapubic port helps in mesh fixation to sacrum. Mesh should wrap rectum 5 cm on each side.
