| Video Discription | a young woman was incidentally diagnosed to have right hydroureteronephrosis on ultrasound. CT scan revealed a functioning kidney with right upper ureteric stricture with proximal dilatation. DTPA showed 44% function with obstructed drainage. Plan was made for retrograde ureterography and proceed. Options of laparoscopic ureteroureterostomy, laparoscopic boari flap ureteroneocystomy, laparoscopic assisted Monti ureteral replacement and laser incision with dj stenting were considered (depending on the findings of RGU). Upon finding short segment stricture patient was subjected to laparoscopic ureteroureterostomy. patient was turned to right flank position and 4 ports were placed (12mm right paraumbillical for camera; 3, 5mm ports one in subcostal, one right lower quadrant and one in flank). After careful dissection upto periureteric tissues without damaging adventitia and artery supplying the postobstructed segment of ureter, a standard spatulated continuous anastomosis performed with 4/0 vicryl (4/0 PDS and 3/0 V-LOC are other options) over DJ stent. Perinephric closure was performed to avoid direct contact of anastomosis with bowel or drain. Histopathology revealed polypoidal ureteritis (rare diagnosis - similar to fibroepithelial polyp). |
