| Video Discription | 45 year old gentleman had been diagnosed with bilateral ureteropelvic junction with small left kidney 18 years back. He had undergone open pyeloplasty of the right side then. in view of asymptomatic status, he was lost to follow up. He presented with elevated creatinine (3.5 mg/dl). CT revealed right large hydronephrotic kidney with UPJ stricture and left small hydronephrotic kidney. Bilateral retrograde pyelography revealed left upper ureteric kinking with no significant obstruction and right severe UPJO with 'jet sign'. Right DJ stenting was performed after which his serum creatinine settled at 1.8mg/dl. He was planned for right laparoscopic / as needed, open dismembered pyeloplasty. The video revealed tips for safe adhesiolysis, reflection of colon and duodenum, dissection of ureter and renal pelvis, reduction of pelvis, spatulation of ureter and ureteropelvic anastomosis. |
