| Video Discription | A sinus meningoencephalocele is tissue that herniates into a sinus cavity through the bone of the skull base. It contains at least the brain tissue lining and sometimes actual brain tissue. If the tissue lining of the brain breaks, and there’s a break in the sinus tissue lining, then patients get brain fluid leaking into their sinuses and then either out the front of their noses or down their throats. We repair these to prevent potentially life-threatening brain infections or air being entrained into the brain cavity. Very reliable endoscopic techniques have been established over the last 3 decades, but one area that has been tricky to access has been the lateral recess of the sphenoid sinus, which resides behind a space called the pterygopalatine fossa. This small space contains a major artery and many little nerves that supply sensation to the cheek and palate. Many surgeons dissect through this space to get direct access to the lateral recess of the sphenoid sinus. While it works perfectly fine to repair the brain fluid leak, it often leaves patients with cheek numbness, partial numbness, or pain issues, which can be pretty bothersome or debilitating to patients. Therefore, I started doing these cases by dissecting around this space, and leaving the big artery and little nerves alone. This technique was published by another group as well, and I completely agree that it is an ideal way to access and repair these types of brain fluid leaks.
