| Video Discription | Intraosseous cavernous malformations can be difficult to excise. Larger lesions usually require an en bloc resection with bone cuts around the lesion. Reconstruction in these cases usually involves the use of patient specific implants. These lesions can have bleeding that is difficult to control and it is important to prepare for that. In this case, the lesion is relatively small and on the inferior orbital rim in the zygoma. This is Richard Allen at oculosurg.com. This video demonstrates excision of an intraosseous cavernous malformation. Review of the imaging shows the lesion at the inferior lateral orbital rim in the zygoma bone. The patient initially had a CT that was read as fibrous dysplasia. An MRI shows that the lesion enhances with contrast, suggesting the diagnosis of an intraosseous cavernous malformation. A swinging lower eyelid approach is used with a lateral canthotomy and inferior cantholysis. A transconjunctival incision is made inferior to the inferior boarder of the tarsus, and dissection then proceeds between the orbicularis muscle and the orbital septum to the inferior orbital rim. A cotton-tipped applicator is used to expose and dissect around the lesion. The Freer periosteal elevator is then used to further dissect around the lesion. The Freer is then used to make an incision through the periosteum inferior to the lesion. Additional periosteal incisions are made with the Freer around the lesion to try to elevate the edges of the malformation. The inferior edge of the malformation can then be elevated and the lesion is mobilized and partially removed. A diamond burr is used to remove any remnant of the malformation from the bone. Any sharp edges of bone are burred down. Burring proceeds until anatomically normal bone is visualized. Due to the depression in the boney rim in this area, the lateral fat pad is mobilized and transposed into the sub-periosteal space over the area of the depression. The fat is engaged with a double-armed 5-0 Prolene suture which then are placed in the subperiosteal space and retrieved after transcutaneous passage of the suture. The fat appears to be repositioned appropriately. The inferior cantholysis and lateral canthotomy incisions are then repaired.
