| Video Discription | Complex aortic revascularization with Aorto-Visceral-Renal Thrombo-Endarterectomy and Aorto-Bifemoral Bypass by infra-colic Transperitoneal approach. Aortoiliac reconstruction with thromboendarterectomy of the aortoiliac and femoral segments was developed first by Dos Santos. Concerns about the technical challenge of thromboendarterectomy and the better long-term patency of aorto-femoral bypass have caused many surgeons to abandon this procedure. In recent years, attention has turned to extend the bypass more proximally to adequately address disease in the Peri-renal and visceral aorta. In contrast to infrarenal aortic exposure, there is little data comparing methods of exposure of the proximal aorta and its branches. Over the last two decades we have utilized two different transabdominal approaches to expose the pararenal aorta; the infra-colic approach and the medial visceral rotation approach. We use the Infra Colic if SMA and both renal arteries require control but the left Medial visceral rotation is utilized in cases that will involve the celiac axis. Left Medial visceral rotation gives us an unrestricted continuous exposure for combined aortorenal celiac reconstruction.
