| Video Discription | Patient had carcinoma of the penis cT2N2M0 (bilateral inguinal nodes). The video shows inguinal nodal dissection on the right side using endoscopic approach. Three ports were used: 10 mm camera port 4 cm distal to the apex of the femoral triangle, 5 mm and 10 mm (initial 5 mm changed to 10 mm for clipping vessels) working ports superomedial and superolateral to the first port respectively. This is part one of the procedure, till the division of the saphenofemoral junction (SFJ). The 10 mm camera port incision is deepened till the deep fascia and then the index finger is used to sweep tissues in that plane to create space. The port is placed and a purse-string suture ensures that there is no gas leak once CO2 insufflation is done. The remaining ports are placed and dissection is carried out in a caudal to cranial direction. The landmarks are the sartorius muscle laterally, the adductor longus muscle medially and the external oblique aponeurosis superiorly. The nodal packet remains suspended from the roof till all dissection is complete in the deeper plane and the SFJ and tributaries are clipped and divided. The remaining procedure not included in this video includes dropping the nodal packet from the roof, division of the great saphenous vein distally and then delivery of the nodal packet in an endobag. |
