| Video Discription | A case record of 7 weeks , pulsating ectopic pregnancy sac implanted at Cesarian scar. Obviously there is deep Isthmocele at CS scar that allowed implantation. Hysteroscooic approach went very nice and excellant view of the sac. Attachment of sac was anterior very close to proximity of bladder. Challenging points were; 1) Very thin ant wall of Isthmocele niche with possible attachment of chorionic tissue to bladder. 2) Restricted space inside cervical canal allowing limited maneuvers by hysteroscope. 3) Caution required for proper irrigation distention to facilitate visibility. 4) Threat of severe bleeding from base as defective muscle wall and lack of contraction in this area Proper use of hydrostatic pressure to loosen sac and create space work around. Judicious push and dissect by Loop electrode implementing combined cold loop push plus electro cautery usage. Push gently in the proper plane at base of sac that is in very close proximity to posterior bladder wall. Wonderful bloodless avulsion of sac from base with very nice final view. |
